PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.

The staff Physical Therapist is required to be flexible with daily scheduling and may be required to work various patient care settings. A staff Physical Therapist is supervised by the Rehab Services Management. Following appropriate practice, a therapist will perform evaluations, determine PT diagnoses, develop a plan of care, and perform physical therapy procedures to aid in restoring function, improving mobility, relieving pain, improving health, and preventing or limiting permanent physical disability. This position may also perform other assigned duties. A staff Physical Therapist must demonstrate the knowledge and skills necessary to provide age, goal, and diagnosis appropriate care. The individual must demonstrate knowledge of the principles of growth and development throughout the life span, be able to assess patients needs and status by analyzing evaluated data, and be able to interpret information to identify age-specific needs and provide appropriate care accordingly per patient care setting. This position must also be able to follow and adhere to the practices described in the department Policy and Procedure Manual.

LICENSURE: Virginia license to practice as a Physical Therapist or traineeship approved by the Virginia Board of Physical therapy. If working/assigned to a PACE facility and Participants; CPR Certification and First Aid training required and must be maintained current throughout employment. Requires proof of current, valid driver's license in good standing with a minimum of 5 years acceptable driving record. Acceptable driving record must be maintained throughout employment.

EXPERIENCE: Minimum of 1 year working experience with the elderly required. Previous healthcare experience preferred.
